Liability waiver

PLEASE READ CAREFULLY BEFORE AGREEING

By attending or participating in the Miracle on Slide Christmas Party, I acknowledge and agree to the following terms:

1. Assumption of Risk

I understand that participation in this event, including but not limited to slides, games, inflatables, recreational activities, walking surfaces, and other event attractions, involves inherent risks. These risks may include slips, falls, collisions, property damage, illness, serious injury, disability, or death. I voluntarily assume all risks associated with my participation and the participation of any minor children for whom I am the parent or legal guardian.

2. Release of Liability

To the fullest extent permitted by applicable law, I release and agree not to hold liable the organizers, hosts, sponsors, volunteers, property owners, vendors, and affiliated individuals or organizations of Miracle on Slide for injuries, accidents, losses, or damages arising from participation in or attendance at the event, including claims based on ordinary negligence, to the extent such claims may lawfully be released. This release does not apply to liability that cannot legally be waived.

3. Responsibility for Children

I understand that parents and legal guardians are responsible for supervising their children throughout the event. I acknowledge that event organizers and volunteers are not responsible for providing individual childcare or supervision.

4. Medical Emergencies

In the event of an injury or medical emergency, I authorize event personnel to contact emergency medical services when reasonably necessary. I understand that I am responsible for medical expenses incurred by myself or my minor children, except where otherwise required by law.

5. Safety Rules

I agree to follow all posted safety instructions and directions from event personnel. I understand that failure to follow safety rules may result in removal from an activity or the event.

6. Acknowledgment and Agreement

By checking the agreement box below, I confirm that:

• I have read and understand this liability waiver.
• I voluntarily accept the risks associated with attending and participating in Miracle on Slide.
• I agree to the release of liability described above.
• I am at least 18 years old.
• If registering minor children, I am their parent or legal guardian and agree to these terms on their behalf to the extent permitted by law.
